We'll help determine whether Dariyan can provide your loved one's authorized services under their Medicaid plan.
STEP 3 — COORDINATE YOUR TRANSFER
If you choose Dariyan, we'll explain the transfer requirements and help coordinate with the appropriate care coordinator or health plan.
STEP 4 — CONTINUE CARING FOR YOUR LOVED ONE
Once the transfer, employment requirements, and service authorization are complete, you may continue providing eligible care for your loved one through Dariyan.
