Our Proven Care Process

Your Initial Consultation is a 60–90 minute in-home visit where our RN or care manager reviews your needs, medications, and daily routines, and completes a quick home-safety check. We explain services, rates, and funding options (private pay, Medicaid/LTC), then outline your individualized Plan of Care, schedule, and start date.

We build your plan around you. After the consultation, our RN/care manager translates your goals, routines, and preferences into a clear, individualized Plan of Care—tasks, schedule, and caregiver match included. We consider cultural and language needs, mobility and safety, and medication routines. Your plan isn’t static: we review it regularly with you and your family, and adjust quickly as needs change

We pair you with caregivers who fit your needs and your personality. We match on skills (ADLs, dementia care, mobility support), schedule, language/culture, and preferences (driver, pet-friendly, male/female). All Virginia caregivers pass background checks, references, work authorization, TB screening, and required training/certifications. You’ll meet your primary and backup caregiver for continuity

Care doesn’t stop at day one. You get routine RN/care manager check-ins, quick care-plan updates when needs change, and 24/7 on-call support for urgent issues. We coordinate with your doctors and family

We don’t ‘set it and forget it.’ Our RN/care manager reviews your Plan of Care on a set cadence (and anytime needs change), checks visit notes through Virginia EVV, and tracks safety indicators like falls, skin issues, vitals, and meds. We coordinate with your physician on new orders, update tasks and schedules, and brief your caregivers—so your care stays aligned with your goals, day by day
